Although global analyses have strengthened our ability to describe where traumatic SCI concentrates and how patterns change over time, improved prediction does not automatically translate into fewer catastrophic outcomes. For road traffic collision (RTC), related SCI, severity and early deterioration are shaped not only by the crash event but also by system characteristics: the road environment and speed management that influence impact forces; variation in vehicle safety and protective equipment; the reliability of prehospital and referral pathways; and social and financial conditions that affect timely access to appropriate care. PRA organizes prevention into three interacting pillars:&amp;nbsp; structural resilience (design that reduces injury severity when collisions occur), social resilience (reliable early pathways that reduce avoidable delays and failures), and cognitive resilience (decision environments that support safer behavior beyond education alone). We conclude with a feasible 12-month action package and minimal indicators that spine and rehabilitation stakeholders can use to prioritize, coordinate, and evaluate low-cost prevention efforts without exaggerating causal claims&lt;/span&gt;&lt;/span&gt;&lt;/span&gt;</abstract>
