Conus medullaris Dermoid cyst is a benign lesion which  usually arise in the lumbosacral  region of ​​the spinal canal and is often associated with congenital spinal dysraphism and dermal sinus tracts. We report a case of a 37-year-old man with progressive paralysis in distal of left lower limb presented to our clinic. The magnetic resonance imaging (MRI) of lumbar spine, as  modality of choice, has revealed a well-defined sausageshape lesion  and multilobular cyst and heterogeneous contrast enhancement of an intra and extra-medullary lision in the conus medullary region. Sub-Total micro-surgical resection was done on the lesion. The result of histopathological examination confirmed the dermoid cyst as well. the patient did  improve after subtotal resection , leading a good clinical result.
