Background and Aim: Symptomatic T1–T2 disc herniations are uncommon, most frequently occurring in a posterolateral location. Posterior approaches, including trans-facet pedicle-sparing techniques, are often used in such cases, whereas other forms require anterior or lateral methods.
Case Presentation: We present a case of symptomatic T1–T2 disc herniation in a 37-year-old patient with diminished hand strength, medial arm and shoulder pain, and paresthesias with radiculopathy due to lateral lesions. The patient underwent posterior-only laminectomy and discectomy and achieved complete recovery.
Conclusion: This case highlights a T1–T2 disc herniation with central/anterolateral lesions that necessitated a posterior-only approach, and follow-up magnetic resonance imaging confirmed full resolution in the conservatively treated radiculopathy case.
